Founded in 1988, Transitions of Long Island is part of the North Shore-LIJ Health System. We offer compassionate and expert outpatient rehabilitation to help people recovering from a neurological injury or illness. Our experienced team of professionals are here to help you or a loved one face this challenge, improve your cognitive, emotional, communication and physical abilities and return a sense of normalcy to your life.

Each of our patients has a unique set of personal challenges. Our team of therapists is specialized in treating patients with neurological conditions. They will work with you to set goals that fit our special needs, ensuring your comfort and successful rehabilitation. Your team may include physical therapists, occupational therapists and speech-language pathologists, in addition to neuropsychologists. Physiatrists (physicians who specialize in physical medicine and rehabilitation), neurologists and pain management specialists are all easily accessible onsite if their services are needed.
A Treatment Plan Tailored to Your Needs
Depending on your condition and goals, your rehabilitation team may help you with:
Self care – eating, grooming, bathing, dressing and basic household activities.
Mobility – balance, strength, range of motion, walking or self-propelling a wheelchair.
Transferring – moving across surfaces (e.g. from a wheelchair to a bed).
Speech & Language – word finding, listening, reading comprehension, intonation, speaking
clearly, volume and swallowing.
Cognitive Skills – improving attention, memory, verbal and visual problem solving.
Social – interacting effectively with other people.
Counseling – improving adjustment, mood and confidence levels.
Support Programs – special wellness series providing education and support is available to you and your caregivers during and after your therapy.
Referrals – programs are recommended to help you identify and prepare for volunteer work and/or new career options.
Communication & Education
Communication is a vital part of the rehabilitation process. The rehabilitation team reviews your treatment plan frequently and provides regular updates to your primary physician. In addition, the team meets regularly with you and your caregivers to review your progress, discuss your goals and review strategies and skills to help you at home.
